The Andrew Durick Foundation is currently seeking individuals who have been diagnosed with Crohn’s and/or Colitis and need financial assistance.
Assistance provided, but not limited to: Doctor, hospital, and medication co-pay deductibles; Medical bills that are not covered by insurance; Medical supplies.
To apply for assistance, please follow the steps below:
1. Download and fill out this HIPAA Authorization for Release of Medical Information Form (PDF), which provides permission for your physician to speak with our Foundation Representative* in reference to condition and account balance.

This offer has no cash value. Payment is issued directly to Physician or Facility where treatment occurred.
